This session shall build on the previous lecture in which Pharmaceutical Care (PC) was introduced, its definitions were discussed, and its main components were illustrated. In the second part, we shall narrow down to discuss history-taking, analysis, assessment components
of PC.
In cooperation with Dr Nadir Kheir, we will continue talking about pharmaceutical care (PC) in hospitals as communication with patients, history taking, assessment, and analysis. Our aim is also to make a case that the PC application in practice will be achieved by explicit and sustainable positioning of PC in the academic area and by forming an alliance with practitioners to fully implement a structured PC of the highest standard in the Middle East.

 Why Pharmaceutical care?

PC seeks to offer new directions to the pharmacy practice as a whole and to re-direct the focus of pharmacists towards individual patients
Pharmacists should, therefore, be directly responsible to the patient for the effectiveness and quality of the care provided
Pharmacists should take a patient advocacy role to protect the patient from the harmful effects of drug misadventure.


The goal of this webinar 

To describe the main patient-centered care elements involved in providing pharmaceutical care with special emphasis on history-taking, assessment, and analysis.

By the end of this lectures, the participants will be able to:
 
1.To discuss the rationale of delivering PC and the candidates for this service
 
2.To define the meaning of patient medication assessment
 
3.To describe the environment and process of history taking
 
4.To list drug-related problems and explain how these are identified and prevented as part of the assessment in PC
